Valid Date of Birth Verifications include:
• State-issued Driver’s License or ID Card
• Birth Certificate
• Passport
• Letter from the Social Security Administration (acknowledged by SSA if applicable)
• Military ID
• Voter ID Card
• Certificate of United States Naturalization (form N-550 or N-570)
Valid Social Security Number Verifications include:
• Social Security Card issued by the government (ie; not plastic or metal)
• Printout from the Social Security Administration (acknowledged by SSA if applicable)
• Medicare Card with any of the following codes: A, J1, J2, J3, J4, LM, M, M1, T, TA
• W2 form
